Daily Health Screening & Tracing

As per Provincial guidelines, ALL Skaters, Coaches, Staff, Volunteers, Parents and Spectators must complete a Health Screener and be recorded on the Tracing Log each day.  The log can be electronic or completed on-site. The electronic version is recommended to help reduce contact and the potential for infection.

MHSC has created an online version combining the Health Screening and Contact Tracing documents. Skaters/Parents can log onto the Google Document no earlier than 3 hours before their scheduled ice time. Be advised that the online form is data and time stamped.

The information collected on the document is being collected to assist in the management of the COVID-19 pandemic.  This information is kept in a secure location and will be provided to Alberta Health Services if it is required for tracing purposes. Neither Skate Canada: Alberta-NWT/Nunavut nor Medicine Hat Skating Club will use this information for any other purpose and all logs will be maintained for 4 weeks and then promptly destroyed. Under Privacy Regulations, all signees have the right to access and correct any information. Results will be downloaded and printed daily or weekly depending on the program class size and the original information source is destroyed/removed from the online portal.
